Acceptable changes on the ACS

Group 1

These are acceptable changes.

The ACS can be processed.

  • Phone and/or fax number
  • Email address
  • Postal address
  • Incorrect title for contacts
  • Incorrect street type (for example, road, avenue, crescent, etc.)
  • Spelling or data entry errors (not changes) to practice name, authorised contact(s), company name, trading name, ownership address
  • Change of name for an authorised contact or owner where that contact/owner has signed the declaration
  • Change of practice name where the declaration is signed by an owner and no other changes are required
  • Updating the current accreditation details. No action is needed. Practices do not need to provide evidence of ongoing accreditation (unless program management requests it) once their practice application is approved. This does not include expired accreditation (see Group 4)
  • Changing to registered incentives (under the Incentive Participation section of the ACS)
    Note: changes to After Hours level, Procedural GP tier, or opting out of eHealth are not acceptable, see Group 2
  • Withdrawing from an individual incentive. Use the date on the declaration as the date of withdrawal
  • End dating a practitioner
  • Adding or updating an ABN
  • Adding a trading or company name where this field was previously blank. This must be signed by an owner
  • Adding or changing a company name or trading name for an individual proprietor. This must be signed by the owner

  • Process the change to the practice details
  • Process ACS

Group 2

These are unacceptable changes.

The ACS can be processed.

  • Changing authorised contact persons (IP005)
  • Change of name for an authorised contact or owner when that contact or owner did not sign the declaration (IP005 or letter from the contact or owner)
  • Changing the practice name when the declaration was not signed by the owner and no other changes are required (letter signed by the owner)
  • Applying for an individual incentive (IP004, IP026 and IP030)
  • Changing the After Hours Incentive payment level (IP030)
  • Changing the Procedural GP tier level (IP003)
  • Opt-out of (not withdrawing from) the eHealth Incentive. Practices must opt-out through HPOS
  • Adding a new practitioner (IP003)
  • A partial ownership change (IP010)
  • Updating/adding a HPI-O number. The practice must update this through HPOS
  • Adding, closing or withdrawing an additional location (IP025)
  • Withdrawing the practitioner from the Procedural GP (IP003)

  • Process ACS
  • Return the ACS to the practice requesting the appropriate forms to update the change

Group 3

These are unacceptable changes.

The ACS can be processed and PIP and WIP – Practice Stream payments must be placed on hold pending the return of the correct form/s.

  • Practice relocation (IP005)
  • Changing the main location address, See Group 5
  • A full change of ownership, or where it is unclear if it is a full or partial ownership change (IP010)
  • Changing the bank account details (IP005)
  • Practice amalgamation (IP005)
  • Closing or withdrawing the main location address (IP007)
  • Changing the trading name, company name, ownership address or where it is unclear if an ownership change has occurred (IP010)

  • Process ACS
  • Place PIP and WIP- Practice Stream payments on hold
  • Return the ACS to the practice requesting the appropriate forms to update the change

Group 4

These are unacceptable changes.

The ACS can be processed.

The PIP and WIP- Practice Stream payments must be placed on hold and issue is escalated to the LPS for further investigation.

If a letter is required, Program Management will arrange it.

  • Amending the public liability to ‘No’
  • Amending the professional indemnity to ‘No’
  • Changing the unit number, suite number, street number, lot number, street name or suburb of a practice if the practice is not relocating. This generally happens because of a council or government decision
  • accreditation details expired

  • Process ACS
  • Place PIP and WIP- Practice Stream payments on hold
  • Escalate to LPS

Group 5

This is an unacceptable change.

The ACS can be processed.

Issue is escalated to the LPS for further investigation.

If a letter is required, Program Management will arrange it.

  • Changing the RRMA
  • Changing the practice type

  • Process ACS
  • Escalate to LPS

Group 6

This is an unacceptable change

The ACS cannot be processed

  • Changing the authorised contact persons when the additional authorised contact has signing the ACS (IP005)

  • The ACS cannot be processed
  • Return the ACS to the practice advising it cannot be accepted

ACS is not processed by 31 July

If…

The ACS and any of the changes to the practice details are:

  • not processed on or before 31 July, if sent through HPOS
  • not returned by 24 July ( if sent by mail)
  • incomplete

The practice's payments will be automatically placed on hold on 1 August.

The practice payments are placed on hold.

The practice payments will be released once the practice confirms the ACS.

Program Management (payments team) will:

  • assess these practices
  • remove the payment hold
  • release any held payment

The Service Delivery team do not need to take any further action.

The PIP payments are withheld for 3 or more consecutive payment quarters.

Program Management (payments team) will withdraw the practice.

Withdrawn practices must reapply to join the PIP.

Exceptional circumstances

Explanation

If the practice:

  • changes ownership
  • relocates
  • amalgamates

The Practice does not need to confirm the ACS.

The practice is confirming their eligibility by changing ownership, relocating or amalgamating.

Confirmation statements are not generated for practices approved in the 6 months prior to the ACS date.

Newly approved practices will not receive an ACS.
