Before the acquisition or merger date

On or after the acquisition or merger date

• Approved Pathology Authority acquisition or merger advice form (HW036) received and assessed

• Response sent to both APAs:
  • If approving - Approval of HW036 application email
  • If refusing - Refusal of HW036 application email
• Approved Collection Centres (ACC) cancellation requests and application for new ACCs received and processed
• ACC cancellation requests assessed by a delegate and approval comment recorded in the Provider Directory System (PDS)
• Accredited Pathology Laboratories (APL) links to the disposing APA end dated and APLs linked to the acquiring APA
• Application for approval, amendment, renewal or cancellation of an approved collection centre form (HW035) not received email template sent (if required)
• Confirmation requested that the acquisition or merger is proceeding as planned (by email 2 business days before the acquisition or merger and by phone on the day of the acquisition or merger if email confirmation was not received previously)

• Applications for ACC cancellations and new ACCs approved by a delegate
• APL contact details amended to those of the acquiring APA
• APA termination requests processed and approved (if required)
• Confirmation of processing email template sent
• Receipt of assignment of lease documents checked - 30 days after the acquisition or merger date
• Reminder email - receipt of assignment of lease documents sent 30 days after the acquisition or merger date (if required)
• Pathology and Diagnostic Imaging Team notified of any outstanding assignment of lease documents (if required)
