Payment suspension notification issued by provider

Shown below is the content of the notification issued by providers when a Non-Attendance Report (NAR) is accepted by Services Australia and payment is suspended. The deemed date of this notification determines the start date of a non-attendance failure penalty when a non-attendance failure is applied for the event date of the NAR.

For letter or email:

Dear [CLIENT TITLE] [CLIENT GIVEN NAME] [CLIENT SURNAME]

You recently missed an appointment with us. Your income support payment has been stopped as a result. Please call us on [phone number of provider] as soon as possible.

For SMS:

Hi [CDP participant first name]. Please call us immediately on [XX XXXX XXXX] as you missed an appt and your payment has been stopped. Thanks, [employment service name]
