More than 3 Polio doses before 3.5 years of age

This section explains how to record more than 3 doses of polio given to children under 3.5 years of age.

If a child under 3.5 years of age has more than 3 polio does recorded on the form, record:

  • the first 3 doses as 1, 2, 3
  • any additional doses as duplicate dose 3 and C and Y each one

If a polio dose was given at birth, or within 7 days after birth, record:

  • the birth dose as dose 1 and C and Y
  • the next dose, where the child is under 1.5 to 2 months old, also as dose 1 and C and Y
  • remaining doses as 2, 3, 3 etc and C and Y

If unsuccessful, escalate to AIR Local Peer Support (LPS)

Actions for incomplete immunisation history form IM013

Service Officers must include any action taken in a processing note in PaNDA, as per Workload Management (WLM) processing guidelines. See Work Optimiser for staff.

IHI number missing

Use name, date of birth and postcode or address search to identify the customer in CDMS, mainframe or the AIR Service Officer Portal.

Customer's personal details missing

Return the form if any of the following details are missing:

  • First name and Surname (unless only name)
  • Date of birth
  • Address

Note: gender is not mandatory

Aboriginal or Torres Strait Islander status indicator not completed

No action required.

This section is not mandatory and requires parental/customer consent to report. The form can be processed.

Hepatitis B birth dose not completed

No action required.

This section is not mandatory and may have already been reported.

The form can be processed.

Vaccination details section - Part B

Section B - Vaccination details

Return the form if:

  • vaccines or doses unclear or
  • dates are incomplete
  • not legible

Batch number, vaccine type or route of administration not completed

No action required.

This section is not mandatory when reporting vaccines given by another provider or overseas

The form can be processed.

Multiple provider numbers supplied

Return the form.

Vaccination details and Declaration date not completed in full

Return the form if any of the following is missing:

  • check box acknowledging the declaration is not ticked
  • date missing - the declaration at Part C must be dated
  • provider details missing

Note: if the provider used a stamp to complete the provider’s name and number, this can be accepted
