Confirm if the customer wants to enrol the newborn in Medicare only + Read more ...

Does the customer want to enrol the newborn in Medicare only?

  • Yes, go to Step 3
  • No, if the customer also wants to lodge an online claim for family assistance, for example, for Family Tax Benefit or Parental Leave Pay, advise the customer to lodge via myGov (unless there is an immediate medical need for a Medicare card, go to Step 7)

Sight completed FA081 and proof of birth + Read more ...

Sight the form and check the following:

  • the customer and partner (if applicable) have signed the FA081 form
  • proof of birth section has been completed by the doctor or midwife
  • official hospital stamp is present
  • customer's Medicare card number is included

Are all sections on the form complete?

Note: if the customer is not the parent, they must be the partner or legal guardian. If the form is lodged by anyone else, Services Australia will:

  • accept the form but not conduct any face to face action or interview
  • scan the form to the Customer Reference Number (CRN), and
  • process the form as part of the Medicare Newborn Manual Enrolments (MAENROL) process, as the Medicare card cannot be given to a third party. See Processing newborn child enrolments in Medicare

Incomplete or missing details or proof of birth on FA081 + Read more ...

If the:

  • form is incomplete or missing details, ask the customer to complete the form
  • proof of birth section is incomplete or missing details, contact the hospital and obtain the:
    • gender of the child
    • mother's name, if illegible
    • date of birth of newborn

Is the form now complete?

Form still incomplete + Read more ...

If the form is still incomplete because:

  • it has no signature from the customer and/or partner, give the form back and ask the customer to obtain signatures and re-lodge via digital lodgement option. Procedure ends here
  • the proof of birth section has not been completed by the doctor or midwife with registration number and contact details and/or no official hospital stamp. If the customer:
    • can provide alternate acceptable proof of birth for the newborn child in the interview, go to Step 6
    • cannot provide the alternative proof of birth, give the form back and ask the customer to obtain proof of birth and re-lodge via digital lodgement option. Procedure ends here

Complete Medicare enrolments and Safety Net registration + Read more ...

Accept the completed FA081.

Does the customer have an immediate medical need for the Medicare card?

  • Yes, go to Step 7
  • No:
    • Tell the customer the Medicare card will be mailed to the group contact listed in the Medicare record
    • Scan FA081 to the customer's Customer Reference Number (CRN) via the Multi-function Device (MFD) in the service centre. Note: if the customer expressly states they do not want to have a CRN created or the FA081 filed on their CRN, complete the enrolment and batch as per current batching processes
    • Store FA081 on site
    • Procedure ends here

Customer has an immediate medical need for the Medicare card + Read more ...

Complete the Medicare enrolment, Medicare Safety Net and My Health Record registration or opt-out as a face to face service, see Process newborn child enrolment to existing Medicare card in CDMS table.

If the customer has indicated they:

Check question 14 on FA081 'Request for partner to be added onto the Family Safety Net' and action if required, see Family registration and confirmation for Medicare Safety Net.

Scan FA081 to the customer's CRN via the MFD in the service centre and close the Medicare activity on the customer's CRN in the Centrelink system.

Note: if the customer clearly states they do not want to have a CRN created or the FA081 filed on their CRN, complete the enrolment and batch in the Medicare Daily Batch envelope. Add comments in CDMS of the batch details.

  • Yes, go to Step 2
  • No, and if all the sections on the form are:
    • complete, scan the enrolment with supporting documents via the Multi-function Device (MFD) to PaNDA.
      Tell the customer the Medicare card will be mailed to the group contact listed on the Medicare record. Procedure ends here
    • incomplete, or missing details, ask the customer to complete the form

Service Officer sights the completed MS004 form and proof of birth + Read more ...

Service Officers must sight the form and check the following:

  • determine if the customer is the applicant
  • check the consent declaration has been completed on the MS004 form
  • ensure all relevant details have been completed and supporting documents have been provided

Where Service Officers identify the form has not been completed in full, and/or the consent declaration provided, return the form to the customer for completion.

Is the customer the applicant?

  • Yes, go to Step 3
  • No:
    • Customer is not the parent, return the form and documentation to the customer and advise that the applicant is to attend the service centre for a face to face interview
    • Customer is the applicant but not the birth mother or legal guardian. Further questioning is required to determine the relationship the person has with the newborn and why they want to enrol the newborn on their Medicare card. Accept the completed MS004 and proof of birth. See Processing newborn child enrolments in Medicare.
