Online claiming

Customers should be encouraged to register and claim online.

Claim forms

Family assistance claim forms need to be completed and lodged by parents, grandparents or guardians caring for children to test their eligibility to receive payments.

Customers can claim online. A Claim for Paid Parental Leave and Family Tax Benefit (FA100) or a Claim for an annual lump sum payment of Family Tax Benefit for the 2023-2024 financial year (FA048) should only be issued to customers who are unable to lodge an online claim.

The Details of your child's care arrangements (FA012) is completed by FTB customers who share the actual care of a child with another person and a pattern of shared care exists over a certain period.

The Claim for Bereavement Payment (FA008) is for claiming a payment after a stillbirth or neo-natal death.
