This document outlines the processing instructions to establish a list of Authorised Healthcare Provider Individuals (auth links) who are authorised to access the My Health Record provider portal on behalf of a healthcare provider organisation.

Who completes form

This form must be completed and signed by an Organisation Maintenance Officer (OMO) or Responsible Officer (RO) of the healthcare organisation.
