Authorised contact person changes for Practice Incentives Program (PIP) and Workforce Incentive Program (WIP) - Practice Stream 012-18102339



This document outlines the process for when a practice changes authorised contact person/s.

Authorised contact person/s

The practice owner/s must nominate one primary authorised contact person to act on behalf of the practice and all additional practice branches. Services Australia will take all documents or HPOS practice updates submitted by an authorised contact person to be on behalf of the practice and the practice owner/s.

The practice owner/s can change the primary authorised contact person at any time. This can be done:

  • through HPOS
  • in writing, Written notification must include:
    • Practice name and practice ID
    • Clearly who is the primary authorised contact
    • Signature of a practice owner registered with the agency

The primary authorised contact person will:

  • receive all communications about the Incentive Programs
  • tell Services Australia of practice changes, and
  • update practice details, including changes to the practice bank account details in HPOS or by submitting the relevant form/s

The practice owners can nominate up to four secondary authorised contact persons to act on behalf of the practice. The secondary authorised contact persons will:

  • tell the agency of practice changes, and
  • update practice details, including changes to the practice bank account details in HPOS or by submitting the relevant form/s

Only the owner(s) can update the following details:

  • change of authorised contact persons
  • change of practice ownership
  • practice relocation
  • practice amalgamation
  • practice closure

Updating authorised contact person/s

The practice owners are the only ones that can update the authorised contact persons.

Practice owners can update the authorised contact person/s through:

An authorised contact person must tell the agency if their family name changes. They must submit correspondence including their:

  • practice name and practice ID
  • former name
  • new name, and
  • signature

Multiple IP005 form changes

A practice can use the IP005 form to notify of multiple changes to practice details. An authorised contact person change may be notified with:

  • a relocation
  • an amalgamation
  • updating practice bank account details
  • ceasing a general practitioner (GP) or nurse practitioner
  • changes to the WIP – Practice Stream health professional details

A practice notifying a change of ownership may use the Practice Incentives Change of practice ownership form (IP010) to notify changes to the practice’s authorised contacts.

Amending/updating authorised contact details

The PIP and WIP – Practice Stream online systems do not have an update or amend function for authorised contact details. Authorised contacts must be removed from the profile and re-added with the correct details.
