Table 1: This table describes a list of forms/modules from A to E which are used in conjunction with the claim forms for ABSTUDY, Assistance for Isolated Children (AIC), Austudy, Pensioner Education Supplement (PES) and Youth Allowance (YA) or forms which are issued when particular information is required.

May also be issued to students and Australian Apprentices when they advise of a major change in their circumstances.
Assurance of Support	Module AR (Assurance of Support) + Read more ... Issue to customers who have entered Australia under an Assurance of Support .This form is not available to the public.
Business (sole trader) or partnership	Module F (Business details) + Read more ... Issue to YA and Austudy customers who are or were, immediately prior to claiming, involved in a sole trader business or partnership.
Compensation	Module C (Compensation and Damages) + Read more ... Issue to customers who have claimed compensation.
Employment	Seasonal, Contract and Intermittent Work Details (SU496) + Read more ... Issue to customers who have engaged in season, contract or intermittent work in the six months prior to lodging a claim.

Organisation details (AIC)	Assistance for Isolated Children (AIC) - Organisation Details (SY067) + Read more ... Issue to AIC organisation applicants for a student in order to obtain information about their care and responsibility, financial institution account details and tax file number.
Overseas	Module O (Residence in Australia and other countries) + Read more ... Issue to customers who have lived overseas or who were born overseas.
Parental income	Module JY (Parent(s)/Guardian details for the Base Tax Year and Current Tax Year) for Dependent Youth Allowance and ABSTUDY customers + Read more ... Issue to ABSTUDY and YA customers who are required to provide parental income information for the base tax year and/or the current tax year. Note: the Advise Parental Income Service can be used by the Parent/Guardian to declare details for the Base Tax Year and Current Tax Year online. This service is only available to parent(s)/guardian(s) of a YA claimant, or for a YA or ABSTUDY customer's parent/guardian when lodging information for a parental income reassessment.

For over 18 year olds issue: SY015 - Youth Allowance or ABSTUDY 'Unreasonable to live at home' Statement by Young Person SY016 - Youth Allowance or ABSTUDY 'Unreasonable to live at home' Statement by Parent(s)/Guardians(s) Note: if the Statement by young person form (SY015) and Statement by Parent(s)/Guardian(s) form (SY016) align as determined by the UTLAH Over 18 Assessment team, the Statement by a Third Party form (SY017) is not required and should not be requested to grant UTLAH (O18). The SY017 is only requested if the Statement by young person form (SY015) and Statement by Parent(s) / Guardian(s) form (SY016) do not align or if a parent is refusing or failing to provide a statement, there is doubt about the reason the young person is unable to live at home, then the Statement by a Third Party form (SY017) will be requested by the UTLAH Over 18 Assessment team in order to process UTLAH
