When does the SY069 need to be completed? + Read more ...

An applicant may be requested to complete the SY069 form to provide evidence that the principal family home satisfies one of the geographical isolation rules to qualify for AIC.

The SY069 may be requested when the distance between the applicant's principal family home and the nearest appropriate state school is close to the AIC qualifying limits.

The applicant needs to complete Questions 1 to 5.

Tell the applicant that the following authorities may complete the rest of the SY069:

  • local bus company proprietor
  • local shire clerk/engineer, or
  • local/state/national roads, water and/or land management authority

Questions 1-5 + Read more ...

The applicant will need to include:

  • their name and date of birth
  • the students name and date of birth
  • the principal family home address

Questions 6-9 + Read more ...

The applicant can assist by advising the name of the nearest appropriate state school.

The authority completing the SY069 should provide the following information:

  • the exact distance (one way) in kilometres from the principal home to the school via the shortest practical route (where there is no transport service to that school)
  • the exact distance (one way) in kilometres from the principal home to the nearest available transport pick-up point, going to the state school by the shortest practical route
  • the exact distance (one way) in kilometres from the transport pick-up point to the school by the route used by the transport service route
  • the time taken to complete the return journey from the principal home to the state school
  • whether or not a closer transport pick-up point could be made available to the family if requested
    • If yes, state the distance to the alternative pick-up point
    • If no, state whether the principal family home is less than 4.5 km (one way) from available transport to another appropriate state school (give name of school)

All third-party authorities must accept the declaration for the form to be accepted and determined by:

  • advising the type of authority or organisation
  • naming their authority or organisation
  • printing and signing the form by hand
  • dating the declaration, and
  • stamping the form with their business or organisation stamp

Tell the applicant they can return the form by:

  • A self service option is available for customers.Online - using the Upload documents service
  • Post - using the address shown on the form
  • In person - at a service centre, or in remote areas, with an Agent of Centrelink field officer

Check for the authority's confirmation of:

  • the name of the nearest appropriate state school
  • the exact distance one way in kilometres from the principal family home to this school via the shortest practical route
  • the exact distance one way in kilometres from the principal family home to the nearest available transport to this school via the shortest practical route
  • the exact distance one way in kilometres from the nearest available transport to the school by the route used by the available transport service
  • the time taken to complete the return journey
  • could a closer transport pick-up point be made available to the family if requested?
  • whether the principal family home is less than 4.5 km from available transport to another appropriate state school

Has the appropriate authority answered questions 6-11?

  • Yes, all provided information can be used for assessment of geographical isolation distance or travelling time. Go to Step 4
  • No, issue a SY069 to the applicant and tell them how to complete the form. If required, see Table 1. Procedure ends here
