Examples of using proof of birth for Centrelink and Medicare

At the time of lodgement, Carol is lodging the form for 'Medicare enrolment only' on the FA081.

Two months later, Carol lodged a claim for Paid Parental Leave.

Use the proof of birth to finalise the claim.

Simona lodged the back page of the FA081 (Doctor/Midwife - Proof of Birth declaration). This is to support a claim for Family Tax Benefit (FTB). A Medicare Details DOC creates.

Medicare staff can use the back page as proof of birth for the Medicare enrolment. The customer may need to give more details on a Medicare enrolment form.

Betty completes the 'Add newborn child' service and uploads the back page of the FA081. The Doctor/Midwife – Proof of Birth declaration has been fully completed, but the top section of the back page is incomplete.

As the newborn child's name has been provided in the 'Add newborn child' service, the back page still serves as acceptable proof of birth for Centrelink purposes.

Susan has participated in the Newborn Enrolment Trial and has been advised by the hospital that they will electronically confirm the birth of her child with Medicare and Centrelink. Susan has been advised due to this she does not need to lodge the FA081 but has called to confirm this information.

Tell the customer to complete the 'Add newborn child' service online or the 'Add child' service via the Express Plus Centrelink mobile app and follow any prompts in this service. See Newborn Enrolment Trial for more information.

  • going to my.gov.au and signing in to your Centrelink online account'

If you cannot use our online services, please fully complete the Newborn Child Declaration and return it to us.

If incomplete FA081 lodged

'Thank you for providing the Newborn Child Declaration however some of the questions were incomplete. To finalise your assessment for family payments, you will need to:

  • complete the 'Add newborn child' service online, or
  • complete the 'Add child' service via the Express Plus Centrelink mobile app'

For PPL/FTB claimants pre-birth not yet processed

If back page of FA081 lodged

'Thank you for providing the back page of the Newborn Child Declaration. To complete your assessment for family payments, we need some more details from you.

Please lodge the fully completed Newborn Child Declaration form including your child's name and details. Or, you can provide your child's birth certificate.'

If incomplete FA081 lodged

'Thank you for providing the Newborn Child Declaration however some of the questions were incomplete. To finalise your assessment for family payments, you will need to:

Please complete the enclosed form and return it in the envelope provided.'

Other acceptable POB lodged and not an FTB current customer

This option is not on the Q777 letter and not available in the Request documents Q777 guided procedure or the RFI in Process Direct.

Use the manual process:

  • Create manual Q777 letter
  • Make sure the letter has a return date
  • Include the Free Text Paragraph option
  • Copy and paste each line of the following information into a separate paragraph:

'Please make a claim by going to:

my.gov.au and signing into your Centrelink account

If you do not have a myGov account, you will need to create one first and then link it to Centrelink'.

Once all variables are entered,

  • print the letter centrally
  • make sure Q777 details are in the Progress DOC on the customer record

Q999

If POB provided other than FA081, but no claim lodged

'Thank you for providing proof of birth for 'child's name'. You will need to make a claim if you would like us to assess your eligibility for:

  • Paid Parental Leave scheme payments and/or
  • family assistance

You can do this by:

  • going to your myGov account

If you do not have a myGov account, you will need to:

  • create one first by going to my.gov.au, and
  • then linking it to Centrelink

If you do not make a claim we cannot determine if you are eligible for payment. For more information on payments available to you, please go to servicesaustralia.gov.au/families and select 'Payments for families'.'

If potential entitlement to Centrelink payment exists
