This document outlines the process for registering, amending or withdrawing a general Practitioner or nurse practitioner in the PIP and the WIP - Practice Stream.

Check the professional indemnityPractitioners must have current professional indemnity insurance to be eligible for registration for the PIP and/or WIP - Practice Stream. Does the practitioner have current professional indemnity cover?

Check provider number detailsCheck the provider number for the main location details on the form). Note: practitioners may complete this section to register a provider number for an additional branch location only. This is acceptable. The main location provider number is not mandatory for these notifications. Practitioners do not need to be registered at the main location. The practitioner details on the form must match the provider number details in the Provider Directory System (PDS) for all locations: Log on to the PDS. Search for the practitioner:
