If creating the letter in:

  • Customer First, select Continuation from the Benefit Status field. Do not select New Claim
  • Process Direct and Customer First, add the following text in the Free Text field:
    Your signed Families Declaration Form. If this signed form is not returned your started claim cannot be assessed and you will need to submit a new claim.
