It is an error when the

  • BSB number
  • account number
  • account name

is omitted or incomplete from the claim form, bank account details have not previously been provided, and there is no evidence to support action taken

EB06 Critical

Claim form not signed

It is an error when the application form is not signed and the form has been actioned

EB07 Critical

Supporting documentation not attached or insufficient

It is an error when the receipt does not include the:

  • description of item
  • amount paid
  • supplier details
  • date of purchase

and there is no evidence to support action taken

EB30 Critical

Verification of details not recorded on file note

It is an error when the details have been verified on the phone and they have not been notated on the file note

EB31 Critical

Program Policy’s approval email not attached for overrides

It is an error when the Program Policy’s approval mail is not printed and attached to front of claim

EB32 Critical

Claimant cannot be uniquely identified from details supplied

It is an error when the claimant details section does not have sufficient information to uniquely identify the claimant, and there is no evidence that contact was made by the Service Officer
