This document outlines information on the Assistance for Isolated Children (AIC) Marginal Distance Confirmation (SY069). This is completed by an appropriate authority to establish entitlement to AIC where the distance or the travel time of the journey from a student's principal family home to the nearest state school is close to the qualifying limits for AIC.
Completing the SY069
In these situations, applicants may be requested to have the SY069 completed by one of the following authorities:
- local bus company proprietor
- local shire clerk/engineer
- local/state/national roads, water and/or land management authority
The applicant needs to complete Questions 1 to 5.
The appropriate authority is to complete the remaining questions, including the declaration which requires all of the following:
- the authority's name
- the staff member's name, contact phone number and signature
- the date
Returning the SY069
The form can be posted to the relevant AIC Smart Centre using the Customer Postal Address on the relevant Office Locator page and addressing the mail to 'Student Services' (this address is also shown on the form). Otherwise, it may be lodged online or at any Service Centre for processing by the AIC Smart Centre.
