Re-order the Confirmation of Concession Card Entitlement (SS055) forms

When a service centre office requires more SS055 concession card forms, service officers must request a new order with their Corporate Support Officer.

The Corporate Support Officer is responsible for ordering a pad of the SS055 concession card forms. They must provide the following information with the order:

• zone cost centre
• full street mailing address
• quantity required (each pad has 50 sheets/forms)
• a brief explanation of the Zone they are servicing

Once the order is complete, service centres should receive the forms within 2-5 working days. Rural delivery can take up to 10 days.
