Carer Payment and/or Carer Allowance - Care Needs Assessment (for a child under 16 years) (SA408)

This is a pre-populated form that can be issued via FCSO with the relevant claim form or via a prompt from the CDTT screen during a Carer Allowance (child) claim assessment or re-issued on the History Summary (HS) screen if necessary. To print a copy identical to the one the carer has, Service Officers can produce a 'Local Print' from the HS screen. The SA408 is available as a system generated form only.
