Acceptable changes on the QCS

- Change of practice name if the declaration is signed by an owner, and no other changes are indicated
- Updating current accreditation details. No action is needed. Practices do not need to provide evidence of ongoing accreditation (unless Program Management requests it) once their practice application is approved. This does not include expired accreditation (see Group 4)
- End dating a practitioner
- Adding or updating an ABN

| Group 2 These are unacceptable changes The QCS can be processed | - Changing the authorised contact persons (IP005)
- Change of name for an authorised contact or owner if the contact or owner did not sign the declaration ((IP005) or letter from the contact or owner)
- Changing the practice name if the declaration was not signed by the owner, and no other changes are indicated (letter signed by the owner)
- Adding a new practitioner or health professional (IP003)
- A partial ownership change (IP010)
- Updating/adding a HPI-O number. The practice must update this through HPOS
- Adding, closing or withdrawing an additional location (IP025)
- Adding a trading or company name where the field was previously blank. This must be signed by an owner and the banking details unchanged (IP010/correspondence from an owner and evidence to support the ownership type)
- Adding or changing a company name or trading name for an individual proprietor. This must be signed by the owner and the banking details unchanged (IP010/correspondence from an owner and evidence to support the ownership type)

- Return QCS to the practice. Advise of the appropriate form(s) to update the amendment(s)

- Changing the bank account details (IP005)
- Practice amalgamation (IP005)
- Closing or withdrawing the main location address (IP007)
- Changing the practice name, trading name, company name, ownership address or where it is unclear if an ownership change of ownership has occurred (IP010)

- Place WIP - Practice Stream payments on hold. If the practice is PIP consenting, put the PIP payments on hold
- Return QCS to the practice. Advise of the appropriate form(s) to update the amendment(s)

Quarterly Confirmation Statement process not completed within timeframe

| The practice's quarterly payments will be automatically withheld by Services Australia if the QCS is not received by the point-in-time date. |
| Practice payments are withheld due to QCS not received | Payments can only be released when all correctly completed outstanding QCS are provided to Services Australia. Program Management (payment team) will monitor these practices, assess, remove the hold and manage the release of any withheld payments. No further action is required by Service Officers after processing the QCS. |
| WIP - Practice Stream payments are withheld and/or zero for 3 or more consecutive payment quarters | Program Management (payment team) will withdraw the practice Any withheld payments may be forfeited.
