Aged Care Means Assessment – Support at Home pre-entry 065-18082330



This document outlines the process for completing a Support at Home means assessment before a care recipient commences care.

Pre–entry Support at Home means assessment

A care recipient can choose to have a means assessment before commencing Support at Home.

Pre–entry means assessment:

  • determines what contribution rate the care recipient needs to pay if they start receiving Support at Home
  • is an assessment of income, assets and circumstances including relationship status and homeowner status
  • will issue a letter to the care recipient and nominee with a summary of the income and assets used to calculate the contribution rates for cost of care and an estimate of their contribution rate if they commence Support at Home

All pre–entry means assessments for Support at Home will be completed using a care recipient's income and assets. Once a means assessment is completed and the means test information is sent to the aged care system, the aged care system will:

  • determine the Support at Home or home care approval date for care recipient
  • apply the correct scheme (Post2014 or Post2025) for a care recipient based on their Support at Home or home care approval date
  • set contribution rates using correct scheme, and
  • issue the correct letter

When a care recipient commences Support at Home, the Post2014 or Post2025 scheme will be populated in Process Direct along with an entry event.

Income from private trusts and companies

When a care recipient has an interest in a private trust and/or private company, they must:

  • complete a Private Trust form (MOD PT), and/or
  • a Private Company form (MOD PC)
  • provide all supporting documentation

Note: these documents are needed for referral to a Complex Assessment Officer (CAO).
