This document outlines information regarding the completion of Mobility Allowance Verification of Work (including voluntary work) and looking for work activities MA005Em, and Mobility Allowance Verification of Training MA005Tm forms.

The documents required to verify a customer’s participation in a qualifying activity for MOB may vary depending on the type of activity.

If the customer is:

Do not request all documents listed in Step 2 or Step 3 as verification. Staff must only request enough information to confirm participation in a qualifying activity.

Note: where a customer has multiple qualifying activities, the customer must be able to verify participation for each activity.

Verifying employment, self-employment, job search or voluntary work + Read more ...

To verify a customer who is working or looking for work, a customer may provide a:

  • Verification of work (including voluntary work) and looking for work activities (MA005Em) form

When an MA005Em has been provided, supporting documents may:

  • be required if the employer/employment service provider or organisation has not completed Part B of the MA005Em, or
  • not be required if the employer/employment service provider or organisation has completed Part B of the MA005Em

Self-employed

  • letter from accountant stating:
    • how many hours customer works in the business
    • if the business is expected to make a profit
  • business plan
  • recent tax return showing business profit or loss details
  • profit and loss statement

Employed

  • recent payslips showing:
    • name of employer,
    • how many hours worked over a 2 and 4 week period
  • letter from employer stating how many hours worked over a 2- and 4 week period
  • copy of recent attendance sheet

Voluntary work

  • letter from welfare, charitable or community organisation stating:
    • how many hours of voluntary work every 4 weeks

Job search

  • customer has a current Job Plan:
    • view Job Plan details to confirm approved job search activities
    • in Process Direct > in the Super Key field, key JOBPLAN > select View Plan
  • customer does not have a current Job Plan:
    • view details provided by the customers employment services provider (for example, Inclusive Employment Australia, Remote Australia Employment Service or Workforce Australia) in Part B of the MA005Em

Verifying vocational training, independent living or life skills training + Read more ...

To verify a customer who is training, the customer may provide:

  • Verification of training (MA005Tm) form

When an MA005Tm has been provided, supporting documents may:

  • be required if the training provider or organisation has not completed Part B of the MA005Tm, or
  • not be required if the training provider or organisation has completed Part B of the MA005Tm

Vocational training

  • enrolment records showing hours required the customer attends over a 4 week period
  • letter from training provider/ organisation stating:
    • the course name
    • expected end date, and
    • how many hours the customer attends over a 4 week period

Independent living or life skills training

  • letter from training provider/ organisation stating how many hours of the course over a 4 week period relate to independent living or life skills

If an MA005Em form is required to verify the customer’s qualifying activity to:

The MA005Em is automatically issued when the 'Details Verified?' field is coded as 'N' on the Employment/Training Details (ETD) screen.

This can occur:

  • at new claim when the customer has not provided proof of employment (including self-employment) or voluntary work or job search activities
  • for current Mobility Allowance (MOB) customers, if changes occur relating to employment, voluntary work or job search activities, and details need to be verified

This form is in a letter style and includes a due date for return. The details of the employer or provider are printed on the form.

  • Part A: shows the details of the customer's employer/service provider we have recorded. These details have not been verified
  • Part B: is to be completed and signed by the employer/service provider - provides details of the employer/service provider and the activities undertaken by the customer

Note: if the employment service provider information is available on the customer record, it is not necessary to issue MA005Em form.

Reissuing MA005Em + Read more ...

In Customer First:

Access to the MOB system by coding 'SVMOB' in the Nxt: field in the ISS system. The ACT: field will then display (MOB) in parentheses.

Go to the Activity List (AL) screen in the customer's record. 'S'elect the ETR activity.

  • 'S'elect the required employer on the Employment/Training (ETS) screen
  • On the Employment/Training Details (ETD) screen, code 'T' in the 'Details Verified?' field
  • Do not finalise the activity. (Do not go to the Assessment Results (AR) screen)
  • Record details on a DOC

If an MA005Tm form is required to verify the customers qualifying activity, to:

  • issue a new MA005Tm, go to Step 2
  • reissue a previously requested MA005Em, go to Step 3

Issuing MA005Tm + Read more ...

The MA005Tm is automatically issued when the Details Verified? field is coded as 'N' on the Employment/Training Details (ETD) screen.

This can occur:

  • at new claim when the customer has not provided proof of training
  • for current Mobility Allowance (MOB) customers, if changes relating to training and details need to be verified

The form is in a letter style and includes a due date for return. The details of the training organisation are printed on the form.

  • Part A: shows the customer's training provider we have recorded. These details have not been verified
  • Part B: is to be completed and signed by the training organisation - provides details of the training organisation and the activities undertaken by the customer

The form must be scanned to the customer's record on the same day of receipt.

For current MOB customers, when the review is uploaded or scanned to the record, the MOB/VER activity will be taken off the Future Activity List (FAL) screen. This makes sure reminder letters are not issued.

In Customer First:

Access the MOB system by coding 'SVMOB' in the Nxt: field in the ISS system. The ACT: field will then show (MOB) in process.

Go to the Activity List (AL) screen in the customer's record. 'S'elect the ETR activity

  • 'S'elect the required training provider on the Employment/Training (ETS) screen
  • On the Employment/Training Details (ETD) screen, code 'I' in the 'Details Verified?' field
  • Do not finalise the activity. (Do not go to the Assessment Results (AR) screen)
