Select person + Read more ... Use the dropdown menu to select the person that the customer wants to view the care plan history of. The page lists:
By default, the customer accessing the information will be selected. Dependants under 14 years are listed in the order they appear on the Medicare card. |

Accept declaration + Read more ... Customers can only view the care plan information for a person under 14 if they:
To accept this, customers must select the Accept declaration button. |

Care plan history displays + Read more ... The My care plan history page includes a summary of the customer or their dependant’s care plans. Customers can select the relevant period through the dropdown menu. Due to how these care plans are serviced:
Note: only 2 years’ worth of care plans will be displayed. |

View claims + Read more ... Customers can select View claims to view claim details for each of the services billed to the care plan selected. |

View details + Read more ... Customers can select View details to see additional information about the service, including the MBS item number and description. Some services may have additional information to advise the customers where the service:
Note: children under 14 years of age will have a restricted view that does not display the servicing provider. |
