My care plan history in Medicare online account (MOA) 133-25040906

• Sign into myGov
• From the Services menu, select Medicare
• From the History and statements menu, select My care plans
• Select My care plan history

Use the dropdown menu to select the person that the customer wants to view the care plan history of.

The page lists:

• the customer who is accessing this information
• dependants who are under 14 years and listed on the Medicare card.

By default, the customer accessing the information will be selected. Dependants under 14 years are listed in the order they appear on the Medicare card.

Customers can only view the care plan information for a person under 14 if they:

• declare they are either the person to whom this information relates, or a parent/guardian who has parental responsibility for the child
• accept that they understand giving false or misleading information is a serious offence.

To accept this, customers must select the Accept declaration button.

The My care plan history page includes a summary of the customer or their dependant’s care plans.

Customers can select the relevant period through the dropdown menu. Due to how these care plans are serviced:

• GPMHTP and CDM plans are listed by calendar year, presented in a Year field
• EDPs are listed by twelve month periods, presented in a Plan duration field
• previous years using the Year dropdown under each care plan to view previous year’s care plan information

Note: only 2 years’ worth of care plans will be displayed.

Customers can select View claims to view claim details for each of the services billed to the care plan selected.

Customers can select View details to see additional information about the service, including the MBS item number and description.

Some services may have additional information to advise the customers where the service:

• was claimed as part of a different care plan, but counts towards the one the customer is viewing
• is included in 2 service groups, but has only been claimed once

Note: children under 14 years of age will have a restricted view that does not display the servicing provider.

Customers can select Download history to download a summary of these services in PDF format.

Customers can select Home to return to the Medicare home page.
