This document explains how customers can view a summary of their Care plan history online using their MOA in myGov.
Care plan service history - general
Customers can view a summary of completed care plan services that:
- they have submitted for:
- themselves, and/or
- a dependant child under 14 years of age listed on their Medicare card
- their health professional has submitted on behalf of:
- themselves, and/or
- a dependant child under 14 years of age listed on their Medicare card
Patients can review the following care plans:
- Chronic Disease Management (CDM) plan
- Mental Health Treatment Plan (MHTP)
- Eating Disorder Treatment and Management Plan (EDP)
Care plan service history for people aged 14-17 years of age
Customers aged 14-17 cannot view their care plan service history online because of privacy and security reasons. See Medicare claims history and claims history statement online for information on how they can access their claims history through Services Australia.
Care Plan service history for children under 14 years of age
If a child aged under 14 years transfers from the Medicare card of one customer to another customer, their care plan history is not, and cannot, be transferred to protect their privacy. New care plan services will appear in the service history going forward. Children under 14 years of age that appear on multiple Medicare cards’ history will display against the card the claim was made under.
