More than 3 Polio doses before 3.5 years of age
If the form has more than 3 doses of Polio, and the child is under 3.5 years of age, record:
- the first 3 doses as 1, 2, 3
- any additional doses as duplicate dose 3 and C and Y each one
If unsuccessful, escalate to AIR Local Peer Support (LPS)
Action for incomplete immunisation history information
Service Officers must include any information verbally confirmed by the provider in a comment in notes as per Workload Management (WLM) processing guidelines. See Work Optimiser for staff.
Circumstance | Action |
Individual personal details missing | Use Medicare card number and reference number to identify a person in CDMS. |
Medicare card and reference number missing | Use name, date of birth and postcode or address search to identify the person in CDMS. |
Personal details missing or the person is unable to be identified | Contact the vaccination provider to confirm the identity of the person, or return the form to the provider with a standard letter. |
Aboriginal or Torres Strait Islander status indicator not completed | No action required. This section is not mandatory and requires parental/individual consent to report. |
Hepatitis B birth dose not completed | No action required. This section is not mandatory and may have already been reported. |
Vaccination details section - no boxes marked, or vaccines or doses unclear | Contact the vaccination provider to get vaccination details, or return the form to the provider with standard letter. |
Batch number, vaccine type or route of administration not completed | No action required. This section is not mandatory when reporting vaccines given by another provider or overseas. |
Vaccinations given overseas | If vaccinations were given overseas, use generic overseas provider number A92009H. |
COVID-19 vaccination given overseas and missing country of administration | Contact the vaccination provider to get the country of administration, or return the form to the provider with standard letter. |
Multiple provider numbers supplied | Contact provider by phone to confirm correct immunising provider or information provider number. |
Declaration date missing | Use date of lodgement shown in PaNDA. |
