Returning Australian citizens who have been absent from Australia for more than 5 years and intend to return to previous country of residence when it is safe to do so, must provide:
- a Medicare enrolment form (MS004)
- valid Australian passports for all customers listed on the MS004
- a statutory declaration stating their intent to reside in Australia for the foreseeable future due to the conflict. Statutory declarations can be approved by Service Officers
If the MS004 is incomplete or documents are missing, Service Officers must escalate the application to Local Peer Support. See Escalation process for Medicare Eligibility.
Note: where some members enrolled on a Medicare card have been absent from Australia for more than 5 years and other members have not, the members who have been absent for more than 5 years must be transferred to a new Medicare card. This is because they will have an entitlement end date. An Application to copy or transfer from one Medicare card to another form (MS011) is not required in this instance.

Once eligibility is established, add the new entitlement details.
On CDMS home page:
- Select Amend Consumer Details
- Key the customer's Medicare card number in ID Number field
- Select Entitlement
- The New/Additional Medicare Entitlement Details will show:
- Entitlement Type Australian citizen
- Country of Relevance (previous country of residence)
- Entitlement Start Date date of arrival
- Entitlement End Date 12 months from date of arrival
Note: the group eligibility start date for Australian citizens has not changed, this remains the citizen’s original eligibility start date.
If an Australian citizen has been absent from Australia for more than 5 years:
- the previous entitlement is required to be end-dated 5 years after the date of departure (if known)
- and the exact date of departure is not known, the end-date is 5 years from the end of the known departure year. That is, 5 years from 31 December of the advised year of departure

Defence Force personnel remain Australian residents for the purposes of the Health Insurance Act 1973. They are not required to satisfy the standard process for residency. This includes if they have been absent from Australia for more than 5 years.

Defence Force personnel must provide the following to enrol or re-enrol in Medicare:
- Completed Medicare enrolment form (MS004)
- Identification, which can include:
- current Australian driver’s licence
- Australian birth certificate
- valid Australian passport
- Residency document, which can include:
- discharge papers
- evidence of employment
- a completed statutory declaration to support their application where documents cannot be provided
