Electronic version of the Mobility Allowance Employment/Training Review + Read more ...

The review form sent to customers selected for an Employment/Training review, advises them to contact the Services Australia Disability, Sickness and Carers line for an electronic version if required.

The Resources page contains a link to 'Phone us' contact information.

Direct customers to the agency's website where an accessible/electronic version is available. It is designed to be used with assistive technology. Once signed and completed, the customer must print the form and return it by the due date.

Customers unable to access the electronic version themselves can be emailed one for completion using their assistive technology.

Does the customer wish to have an electronic version of the MOB Employment/Training review form emailed to them?

Note: customers can register to receive alternative letter formats.

Electronic version of the Mobility Allowance Review + Read more ...

If the customer requests an electronic version of the MOB Employment/Training review form be emailed to them for completion using their assistive technology:

  • confirm the customer's current email address and update if required
  • tell the customer:
    • the fillable MA003MY form will be emailed to them
    • they must not return the fillable MA003M via the return email. The form must be completed, signed, printed and lodged via normal procedures
  • email the Electronic Document Classification (EDC) Team with the following customer details:
    • name
    • Customer Reference Number (CRN)
    • email address
  • create a DOC on the customer's record stating the date the customer contacted and requested an electronic version of the MA003M be emailed to them

The EDC team will email an electronic version of the Review of Mobility Allowance (MA003Y) form to the customer’s email address from the EDC team mailbox:

  • check the return date for the MOB/ETR Employment/Training review on the Future Activity List (FAL) screen
    • V’iew the activity
  • tell the customer to return the MOB/ETR Employment /Training review form by the due date and Create a DOC on the customer's record noting:
    • the date the electronic form MA003M was emailed to the customer
    • customer has been advised the date the form is to be returned

Reminder MOB Review form received by Electronic Document Classification (EDC) team + Read more ...

If a reminder Review of Mobility Allowance form (check History Summary (HS) screen if form is a reminder form) is received at the EDC team Manager mailbox, the Service Officer is to forward the original email to the customer’s email address.

Check the return date for the Reminder MOB/ETR Employment/Training review on the Future Activity List (FAL) screen:

  • V’iew the activity

Tell the customer to return the MOB/ETR Employment /Training review form by the due date and Create a DOC on the customer's record noting:

  • the date the reminder electronic form MA003M was emailed to the customer
  • customer has been advised the date the form is to be returned

General information on Medical Reviews + Read more ...

Medical reviews are only issued to customers who have the Inability to Use Public Transport: field on the Mobility Misc. Circumstances (MOCD) screen coded as 'C' (conversion) or 'E' (extended period). The Medical Report - Mobility Allowance Review (MA002R) is issued if the customer is selected for a review.
